È il 1339 e il popolo reclama una guida che non appartenga alla nobiltà. Quella guida è il plebeo Simone Boccanegra, membro d'una famiglia mercantile. Il popolo lo invoca come nuovo Abate e lo acclama Doge, un titolo che a Genova non esisteva. È il primo doge di Genova e rappresenta il volto di una città potente ma divisa, sospesa tra ambizione politica, conflitti interni e apertura sul mondo. Un personaggio che nell'opera di Verdi diventa non solo un governante ma un uomo solo, consumato dal potere e attraversato da un desiderio quasi impossibile di pace e riconciliazione. Una figura estremamente moderna che Passato e Presente racconterà andando sui luoghi della Genova medievale con Maria Sole Sanasi. In studio con Paolo Mieli il professor Antonio Musarra.

Il primo caso è quello di Inga e Michael, che la mattina del loro matrimonio subiscono un paradossale incidente: mentre sta facendo uscire dal parcheggio l'automobile d'epoca per andare in chiesa a sposarsi, la promessa sposa investe il suo partner e gli rompe una clavicola. Dopo la visita, Michael viene autorizzato a sposarsi, e a farsi operare in seguito la frattura: anche perché ha detto, mentendo, di aver bevuto e di non poter prendere antidolorifici. Preso dal panico, Michael dice a Inga, la quale non affatto è intenzionata a rimandare il matrimonio, di non volersi più sposare. Inga ha un malore, e si scopre che la ragazza ha già avuto un aneurisma, ed è per questo motivo che ha perso il controllo dell'auto poco prima. Dopo l'intervento di entrambi, quando tutto sembra compromesso, il loro amore a poco a poco rifiorirà, come in una telenovela che si svolge sotto gli occhi dei loro medici curanti. Il secondo caso è quello dell'arborista Jonas, ex compagno di scuola di Elias, all'epoca studente popolare e brillantissimo. L'uomo cade sotto gli occhi di Elias da un albero su cui era svenuto, e presenta subito una tosse fortissima e la febbre alta. Elias non riesce a capire dalle analisi del sangue l'origine dell'infezione che provoca quei sintomi: perfino l'aiuto del suo mentore, il dottor Lindner, non porta ad alcun risultato.
